About Compounded Medications

Some medications available through California Trim Clinic may be compounded. Compounded medications:

  • Are made by licensed pharmacies
  • Use pharmaceutical-grade ingredients
  • Are not FDA-approved but may be prescribed when FDA-approved alternatives are unsuitable or unavailable

They are tailored to individual patient needs, including specific dosages, delivery methods, or combinations.

GLP-1 Treatments (Semaglutide, Tirzepatide)

Compounded Semaglutide

Semaglutide is used under medical supervision to support weight loss in patients with or without Type 2 diabetes. Side effects can include:

Purpose: Used to support weight loss and metabolic health under clinician guidance.

  • Nausea, vomiting, diarrhea, or constipation
  • Abdominal discomfort
  • Risk of gallbladder issues or pancreatitis
  • Rare but serious: thyroid tumors, vision changes, or allergic reactionsconstipation

  • Gallbladder disease signs (upper abdominal pain, fever, jaundice)
  • Thyroid C‑cell tumor risk
  • Potential hypoglycemia
  • Vision or mood changes

Compounded Tirzepatide

Tirzepatide combines GLP-1 and GIP pathways and is prescribed for enhanced weight loss outcomes. Possible side effects include:Special Considerations: Avoid alcohol, and do not use while pregnant or breastfeeding. Monitor organ function.

  • Nausea, GI discomfort
  • Dehydration or kidney strain
  • Fatigue or injection site irritation
  • Hypoglycemia, especially with other diabetes medications

Note: As with Semaglutide, compounded Tirzepatide is not FDA-approved. It is only available via prescription and clinician oversight.

Compounded Retatrutide

Compounded medications may be prescribed by licensed clinicians but have not been evaluated by the FDA for safety, effectiveness, or quality. FDA-approved medicines containing retatrutide are not yet commercially available.

Important Warnings

  • Thyroid C-cell Tumors: Retatrutide has a boxed warning for potential risk of thyroid C-cell tumors. Do not use if you or a family member has a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Pancreatitis: Seek immediate care if you experience persistent severe abdominal pain, with or without vomiting.
  • Gallbladder Disease: Symptoms such as upper abdominal pain, fever, jaundice, or pale stools may indicate gallbladder issues.
  • Kidney Complications: Dehydration from nausea or vomiting can worsen kidney function.
  • Severe GI Disease: Use caution if you have a history of severe gastrointestinal issues.

When to Stop Use & Call Your Provider

  • Difficulty swallowing, hoarseness, neck swelling, or shortness of breath
  • Severe abdominal pain or persistent nausea/vomiting
  • Mood changes, suicidal thoughts, or depressive symptoms
  • Rapid heart rate or vision changes
  • Severe allergic reactions (swelling, rash, difficulty breathing)

Drug Interactions

  • Retatrutide slows gastric emptying, which may affect how oral medications are absorbed.
  • May increase risk of low blood sugar when used with insulin or sulfonylureas.
  • May reduce effectiveness of birth control pills—use backup contraception as advised.

General Guidance

  • Not for use during pregnancy or while breastfeeding. Stop use at least 2 months before trying to conceive.
  • Alcohol use is discouraged while on this medication.
  • Always follow dosing instructions as prescribed by your clinician.
  • Never share your medication with others.
